Active Covid outbreak sites info

Hi! It would really help up if you included the location (City + Street) for businesses listed as Active Covid outbreak sites. I try to avoid going into a business if it is an active site. Also, re Sam's Clubs - you show # as 6333, but the FC Sam's Club is #6633, so should the store # be corrected?

Thanks for your input. We get the list of outbreaks from the state and manually enter addresses and sometimes establishments with multiple locations are hard to distinguish. The address has been added.
